What to Expect at Initial Appointment at the Mercy Center for Memory Health
Changes in memory can be challenging for both the person experiencing them, as well as their care partner and/or family members. Fortunately, Mercy’s Center for Memory Health is experienced in evaluating and diagnosing memory concerns.
Prior to the first appointment, a nurse navigator will call the patient and/or their caregiver to discuss the upcoming consultation. The staff at the Center for Memory Health encourage care partners to attend the appointment, as well, which can last up to 90 minutes.
Upon check in, both the patient and the caregiver will complete individual surveys regarding how they’re feeling. A nurse will then spend approximately one hour with the patient and caregiver. During this time, she’ll collect and document an extensive amount of information that may contribute to changes in memory. In particular, plan to bring a list of current medications, including dosage and frequency. It’s important that your healthcare team knows everything you’re taking to ensure there are no drug interactions.
Among other topics, the nurse navigator will inquire about the patient’s:
- Symptoms and overall physical health
- Daily routine and sleep schedule
- Ability to manage various life skills
- Ability to care for him or herself in a variety of situations
- What brings you joy
The nurse navigator will then administer a variety of tests to measure the patient’s cognitive functioning. Then, after reviewing the patient’s results and unique situation, the nurse practitioner or neurologist will spend approximately 30 minutes with the patient and caregiver. During this time, she or he will explain the results of the tests, including how the patient scored compared to what is considered normal aging. They’ll also identify normal findings versus any areas of concern. If necessary, they’ll also give a diagnosis during the appointment and make any recommendations to create an individualized plan.
Finally, the memory nurse will return with a personalized notebook that has additional information regarding the diagnosis, as well as tips to maximize brain health and meaning and purpose in life.
