You may also access and request copies of your medical record through MyChart. Simply request the documents you need and they will be securely released to you via your MyChart account.

Guidelines For Medical Records Requests

All requests for copies of medical records must be received in writing. If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign the authorization. Here are the options to request medical records:

• Print a copy of the Release of Health Information Authorization form (PDF) or en Español (PDF). Please note: There may be a charge associated with copies of the medical record. This completed and signed form may be faxed to (319) 398-6848, mailed to the Health Information Management Department at the address to the left, or emailed to ROI@mercycare.org.
• Request records through MyChart. This can be found by going to Menu, My Record and Request Records. Simply request the documents you need, and they will be securely released to you via your MyChart account.

Request a Copy of a Birth Certificate

Patients born at Mercy who wish to obtain a copy of their birth certificate must contact the County Recorder at:
