1. Enter the ramp from either 8th Avenue or 8th Street.
2. Park on Level 3, near the South building entrance.
3. Enter the building by the South building entrance.
4. Once inside, follow the hallway past the elevators until you reach the Birthplace entrance.

2. Parking is available immediately outside of the entrance along 8th Street.
3. Cardiac & Pulmonary Rehabilitation is located in Suite 102.

Located on the Ground Level. Park in the 8th Street ramp.

1. Enter the ramp from either 8th Street or 8th Avenue.
2. Park on the Ground Level.
3. Enter through the North entrance.
4. You will pass through one set of doors, past the elevator (on your right), and through another set of doors that open near an information desk.
5. The office will be through the door directly ahead of you.

Located on the Ground level, off of 8th Avenue. You may park either in the 10th Street Ramp or in the Emergency Department parking lot.

To enter from Emergency Department parking lot:

1. Drive into lot from 8th Avenue.
2. Park in “Patient” parking spot.
3. Enter the hospital through doors under large “Emergency” sign.
4. Turn right to enter the Emergency Department.

To enter from 10th Street Ramp:

1. Drive into the ramp from 8th Avenue or 10th Street.
2. Park on the Ground Level or Level 1.
3. Exit the ramp through the southern (nearest to 8th Avenue) stairwell or elevator.
4. Cross Emergency Department parking lot.
5. Enter through sliding doors under the “Emergency” sign.
6. Turn right to enter the Emergency Department.

Located on Level 2. Park in the 8th Street Ramp.

1. Enter the 8th Street ramp from either 8th Avenue or 8th Street.
2. Park on the Level 2, nearest to the north building entrance.
3. Enter through the north entrance.
4. Walk past the elevators.
5. Endoscopy will be the first right past the doors.

1. Park in front of the Hall-Perrine Cancer Center, located at 701 10th Street SE.
2. Enter through main entrance.
3. Turn right and walk all the way down the hallway with glass windows.
4. Turn left. The Especially For You office will be straight ahead at the end of the hall.

1. Enter the Hiawatha Medical Park building on the Boyson Road side.
2. Upon entering the building turn left and proceed to the bank of elevators on your left.
3. Take the elevator to level 3 for the Surgery Center.
4. The Information Desk will be at the front of the waiting room you enter off of the elevator.

   Third floor: Medical and Hematology Oncology

Located on Level 1 near the cafeteria. Park in the 10th Street Ramp.

1. Enter the ramp from either 8th Avenue or 10th Street.
2. Park on any level.
3. Exit the ramp through the north stairway or elevator.
4. Enter the hospital building through the 10th Street Entrance.
5. Follow the entrance corridor to the Information Desk.
6. Turn left at the desk, making your way toward Uncommon Grounds coffee shop.
7. Turn left at Uncommon Grounds.
8. Hallagan Education Center will be the second doorway on your right.

Located on the Ground level, off 8th Avenue and near Lundy Pavilion. You may park in either ramp.

To enter from 10th Street Ramp:

1. Drive into the ramp from 8th Avenue or 10th Street.
2. Park on the Ground Level.
3. Exit the ramp through the southern (nearest to 8th Avenue) stairwell or elevator.
4. Cross the Emergency Department parking lot to enter Lundy Pavilion.
5. Turn right into the hallway.
6. Proceed ahead to Radiology, which will be about halfway down the hallway on the right side.

To enter from 8th Street Ramp:

1. Drive into the ramp from 8th Street or 8th Avenue.
2. Park on the Ground Level.
3. Enter the hospital building through the South (nearest to 8th Avenue) entrance, into Lundy Pavilion.
4. Walk toward the large “Emergency” sign at East end of building.
5. Walk under the Emergency sign and turn left into the hallway.
6. Proceed ahead to Radiology, which will be about halfway down the hallway on the right side.

To enter from 8th Street Ramp:

1. Drive into the ramp from 8th Avenue or 8th Street.
2. Park on any level of the ramp.
3. Enter the building through the North Entrance.
4. Take the North Elevator, just inside the doors, up to Level 5.
5. From Level 5 proceed into the building.
6. The Behavioral Unit will be on your left.

To enter from 10th Street Ramp:

1. Drive into the ramp from 8th Avenue or 10th Street.
2. Enter the building through main entrance on 10th Street.
3. Take the elevator to Level 8.
4. The Pediatric Inpatient Center entrance is on the right.

*Note: This is a locked floor. Please use the phone to call the Nurse’s Station for entry to the unit.

Located on Level 4. Park in the 8th Street Ramp.

1. Enter 8th Street Ramp from either 8th Avenue or 8th Street.
2. Park on the Level 4, near the north entrance.
3. Enter through the North entrance.
4. Once inside the building, walk past the waiting area.
5. Turn left.
6. Proceed down the hall; the check-in desk for the Pain Center will be on your left.

To enter from 8th Street Ramp:

1. Drive into the ramp from either 8th Avenue or 8th Street.
2. Park on the Ground Level.
3. Enter the building through the South Entrance.
4. Once inside, walk into Lundy Pavilion and turn left down the hallway.
5. The Katz Cardio/Cath Lab will be on your right.

To enter from 8th Street Ramp:

1. Drive into the ramp from 8th Avenue or 8th Street.
2. Park on the Ground Level.
3. Enter the building through the North Entrance, near Courtyard.
4. Once inside the building, turn right.
5. Follow the red hearts on the wall.
6. The Lung Center will be on your right.

To enter from 8th Street Ramp:

1. Drive into the ramp from 8th Avenue or 8th Street.
2. Park on Level 1.
3. Enter the building through the North Entrance.
4. Walk straight through the doors, past the Information Desk and the “C” Elevators.
5. The Patient Advocate’s office will be on your left, just past Elevator C, before Volunteer Services.

To enter from 10th Street Ramp:

1. Drive into the ramp from 8th Avenue or 10th Street.
2. Park on any level.
3. Exit the ramp through North elevator or stairwell.
4. Enter the building through the 10th Street Entrance.
5. Follow the entrance corridor to the Information Desk, and turn right.
6. Elevator B will be directly ahead of you.

3. Enter the building through the South entrance, marked “Pediatric Clinic.”
4. Once inside the building, proceed straight ahead and turn left.

To enter from 8th Street Ramp:

1. Drive into the ramp from 8th Avenue or 8th Street.
2. Park on Level 2.
3. Enter the building through the North Entrance.
4. Take a right down hallway.
5. PASE will be on your left.

Located on Level 2. Park in the 8th Street ramp.

1. Enter the ramp from either 8th Street or 8th Avenue.
2. Park on Level 2, near the South entrance.
3. Enter through the South door, which leads directly to the Surgery Center.

Located on Level 1 near the Mezzanine and above Lundy Pavilion. Park in the 8th Street Ramp.

1. Drive into the ramp from either 8th Avenue or 8th Street.
2. Park on Level 1 near the South building entrance.
3. Exit the ramp into the building through the South door (nearest 8th Avenue).
4. Walk past the elevators (which will be on your left).
5. Take a left down the first hallway, just past the doll houses.
6. The door to the Women’s Center will be the first door on your right.

To enter from the 8th Street Ramp:

1. Drive into the ramp from 8th Avenue or 8th Street.
2. Park on the Ground Level.
3. Enter the building through the North Entrance, near the Courtyard.
4. Follow the hallway to the left of Dr. Abernathy’s office.
5. The hallway will end near Elevator B; turn right toward the Radiology Department.
6. The Wound Center will be on your right.
