All requests for copies of medical records must be received in writing. If the patient is a minor (younger than 18), a parent or legal guardian must sign the authorization.

Please download the authorization form and contact your provider's office for information regarding release of your medical records. 

Please note: There may be a charge associated with copies of the medical record.
